Keeping a simple wellness journal
Record everyday patterns to support a clearer conversation at your next consultation.
Keep the notes manageable
A short daily note can be easier to maintain than a detailed diary. Record the date, the concern you noticed, and how it affected your day. Use your own words; the aim is to communicate clearly, not to diagnose yourself.
Notice context, not just symptoms
You might note sleep times, meals, activity, medicines and stressful events alongside your concern. Record changes in routine and any treatment changes recommended by a clinician.
Patterns can be useful discussion points, but two things happening together does not prove that one caused the other. Bring observations to your appointment rather than making major treatment changes on your own.
Bring a brief summary
Before the appointment, choose a few examples and write down your main questions. Mention whether a concern is new, worsening or disrupting daily activities. A concise summary helps leave room for the doctor's questions.
Protect your privacy
Wellness notes can contain sensitive personal information. Keep them somewhere private and share only what is useful for your care. If tracking makes you anxious or takes over your day, discuss a simpler approach with your clinician.
